In consideration of the opportunity afforded me to assist on a voluntary basis at Sunnybrook Community Church, related programs and Sunnybrook Hope Center, collectively “Sunnybrook”, where physical labor, lifting, clean-up work, and aiding in programming by volunteers at one of more facilities, I hereby waive any right, claim or cause of action arising as a result of my participation in acting as a volunteer from which any liability may or could accrue against Sunnybrook Community Church or Sunnybrook Hope Center, or any other organizer or supervisor of a volunteer effort, including other volunteers, individually or collectively. Without limiting the generality of the foregoing, I agree that this waiver shall include any and all rights, claims or causes of action resulting from personal injury to me or damage to my property sustained in connection with or during my activities for Sunnybrook.
